• Trade name, occupation or specialty performed by employees and their apprenticeship period
• Precise start time, breaks and end of work, regular hours, time-and-a-half and double-time hours for each work day, for each job site and for each client (name) with whom the employer has a contract
• Nature of the work, type of job site and job site address
